The CMS-1500 (Form 1500) is the standard paper claim form used to bill Medicare, Medicaid, and most commercial payers for professional medical services. It captures patient demographics, insurance information, diagnosis codes, procedure codes, charges, and provider identifiers required for claim adjudication.
Use the CMS-1500 for professional claims billed by physicians, nurse practitioners, physical therapists, and other individual providers in outpatient settings. The UB-04 (CMS-1450) is used for institutional claims from hospitals, skilled nursing facilities, and other facility-based providers.
The form is organized into key sections: patient and insured information (Boxes 1–13), physician/supplier information (Boxes 14–33), and diagnosis/procedure detail (Boxes 21–24). Errors in Boxes 1a, 21, 24, 25, 27, 31, and 33 are among the most common causes of claim rejection.
Most claims today are submitted electronically as ANSI X12 837P transactions. The CMS-1500 fields map directly to loop and segment data in the electronic format, making accurate paper form knowledge essential for troubleshooting EDI rejections.
